HOME HEALTH

Our Home Health division provides both home health and home care. Our teams provide personalized healthcare to clients of all ages in the privacy of their home. We collaborate with physicians, hospital discharge planners, care managers and other healthcare professionals to coordinate the care for our clients.

Our experienced home health team of healthcare professionals includes Registered Nurses, Licensed Practical Nurses, Certified Nursing Assistants, and Personal Support Specialists. We provide both nursing and personal care services to those living with chronic health needs in the home. Our Home Health program is for children under the age of 21 and no prior assessment is needed to start services. Once you call or are referred to our office, we will assign a nurse to assess your child’s needs in the home. Based on the assessment and level of care needed, our nurses will make recommendations to the state for approval.

Home Care Services

Our experienced home care team works with adults living with chronic health needs over the age of 21. Our caring team of Personal Support Specialists (PSS) provides basic home care needs. In our home care program, adults need to have been previously assessed by Change Health Care, formerly known as the Gould Assessment. Once you call or are referred to our office, our Home Care Coordinator will assess your situation and assign a PSS to your case.
